Creating Individual Health Plans (IHP) for Students

Creating Individual Health Plans (IHP) for students with chronic and acute conditions is crucial work of the School Nurse. The IHP is guided by the nursing process, assessment, nursing diagnosis, outcomes, planning, and evaluation.


Following the Nursing Process assures the nurse provides high-quality, patient-centered, evidence-based care. School Nurses write IHPs to guide the care they provide in school. Creating effective IHPs necessitates continual education.


Friday, April 7, 2023, KESD's nurses received up-to-date education from one of the nation's top IHP educators--Sandi Delack, M Ed, BSN, RN, NCSN, FNASN. In addition to more than 30 years as a school nurse, Ms. Delack is past president of the National Association of School Nurses and Past President of the National Board for Certification of School Nurses.
